It’s because setting “exclusive mode” on a DirectInput device ties it up with a window. The “windows” MAME uses in D3D exclusive full-screen mode aren’t suitable for this purpose, and to make it worse, MAME pulls the window out from under DirectInput when you toggle full-screen mode. MAME’s always been badly behaved in this regard, it’s just the other recent changes mean it’s more likely to crash for some people now than it was previously.
Not using DirectInput controllers avoids the issue because MAME won’t do bad things with DirectInput in that case. Not using D3D full-screen exclusive mode and also not hitting Alt+Enter avoids the issue because MAME will use a normal enough window and not rip it out from under DirectInput unexpectedly.
