It's the database of removable media for the systems, so cartridges, disks.
So basically it tells MAME what ROMs are required for each game, what's in the cartridges, how it should map etc. when they get inserted. It's a bit like the internal database of arcade titles, but since there's so much of this stuff, it's stored in software list files.
