sometimes things like -video make a difference.
I had a core 2 duo laptop with an nvidia card which could use motherboard ram, but it was insanely slow. When using direct3d MAME ran fast until you did something like pressing tab or press f11 to show the frameskip.
Back then -video ddraw was an option, but that has unfortunately gone now. You can do -video gdi though, or -video opengl
If you find the last version of mame that ran fast (without any mame.ini, so it's using defaults) then that will give you a clue. If you're not using defaults, then try using defaults.
