I see alien head! I was baked when I saw this, but I still easily see the dog's nose is the alien's ear, the dog's ear the alien's mouth, the perspective being from off and behind the 'operating table'.
And, a head in the clouds on the right! Actually, a little obscure, but visible enough, mouth is lower right, and the head is looking to the right of the viewer.
