Went through my PCB photo collection, and Chicago Coin's TV Hockey is an exact match, aside from the PCB color. Switch placement is identical. No other Pong clone PCB photo I have looks identical. So it's either an original or a clone. http://discrete.mameworld.info/Rough/Rough.htm
Manufacturers with that blue PCB in my collection include: Pele Soccer (unknown mfg.) Play Time (Midway) Video Five (United Games) Trapshoot (Gremlin) and some Sega and Atari discrete games look damn close to this shade blue...
