A few things to check:
-Make sure you are running 0.149 from MAMEDEV.org
-Make sure all files located in HLSL folder are overwritten with what is provided in the binary download. Attempting to use mismatched HLSL files will most certainly cause failure
Those those don't help, you need to run mame with the -verbose output and capture that text to paste here to help us help you.
