I don't think you can have per-monitor rotation settings, at least I don't know how to do it. What I would do with your setup is keep numscreens at 1, and set "screen" to \\.\DISPLAY3 (or whatever your best screen is). You can either play everything on that screen, or you could create a vertical.ini, override "screen", and pipe the vertical games to your vertically oriented monitor.
