Mechanical games and artwork.

I know MAME is not designed to add artwork for game play. (as in 18 wheeler). Would there need to be a major change to MAME's core? I just thought that it would be nice to simulate gameplay for some of those games...If artwork could be found.

How much would MAME need to change?
