Before filing a bug report with MT, I would recommend downloading 143 binary and extracting into an empty directory or even secondary HDD if possible.
execute mame -cc to create a fresh mame.ini then set your paths/options to your preference.
Crashing 50% of the time and the end result not being consistent leads me to believe it's a software/hardware problem on your end and not an actual bug that would warrant a report.
