15Khz will not work if the ArcadeVGA driver is not installed. 24 and 31Khz modes will not work because your monitor (probably) doesn't support them.

You should not install any ATI drivers. Install the ArcadeVGA driver.
It's possible that your arcade monitor is switching itself off automatically because the signal you're feeding it is out of range. At least it's not broken, it works with the Blood Bros PCB.
I'm pretty sure (I don't own an ArcadeVGA) that you will not be able to use your arcade monitor and your PC monitor at the same time, no matter if the drivers are installed or not. Unless your arcade monitor has 31KHz support. Look for a jumper or switch on the monitor PCB but be careful.
