Started with @Nightvoice's file then found an image of the Instruction card and added woodgrain and labels to the Control Panel to match the cabinet pics posted at

Also found what looks to be an original Instruction card on the Launchbox games DB, no way to tell so added an Instruction Card View

Also You have to set the UPRIGHT CONTROLS to Duel in the games DIP switch settings in MAME or player 2 controls will not work in the game.
