It is a pain. It probably has to some different issues. It could be some sort of law 'do not put xyz in your game' or 'you can only sell games that are rated bellow a particular rating'. It could also be the way the game was financed. Movies do this. Maybe the your region has financing run through a particular group. So any copies sold in that region get the profit or portion of it. It is why sequels for movies are inconsistent. A movie may bomb in one region but be a major hit in another. So when it comes time to finance a second version one region is like 'hell yeah' the other one is 'I lost money on that last time... skip' yet you need both to make it. tl;dr laws or contracts.
From a consumer PoV it is just a pain and a waste of my time.
