Instructions how to correctly map analog inputs in Mame?

I am trying to map analog controls correctly in Mame. I cannot find any good documentation on this.

I am interested in information on the native Mame menu that comes up with "TAB"

My main questions are:
- what keys control the input menu? How do I
- reset to default
- erase a mapping
- map multiple inputs
- map only a single input (sometimes my analog trigger is registered as an analog axis AND a digital button)
- map analog controls correctly
- How can I ensure my analog controls are treated as truly analog without any digital-to-analog conversion? For example, it seems like the digital speed setting in the analog controls menu still affects my analog controls, even if I leave the _inc and _dec settings at "none". My understanding is that the speed setting is only for digital-to-analog conversion and I would like to switch it off completely.
- Should I map the x+ or the x- axis to analog controls? I understand that the mapping should be an axis, not a + and -. Again I don't want digital-to-analog, I want real analog controls.
