A lot depends on the individual game and what you want it to look like and the size/resolution of your monitor. The hlsl_prescale settings seem to make a big difference. These are the settings I'm using for Pac Man on a 42" TV at 1920x1080, which are the defaults I use for all games. But different games require different tweaks in the scanline params depending on if they have lower resolution or are in monochrome, etc.
