Actually for the power on step, I was thinking of entering an access code as well. Not everyone has their phone set up this way, but many, including me, do. I believe Apple even recommends setting one.

I don't know what other people do, but I almost always shut my phone off with an app loaded. I'm generally using my phone while doing something else (mostly riding the subway or pooping, but usually not at the same time), so when it's time to go, I just power it off and stick it in my pocket. It's not a huge delay, but it sometimes takes a second or two, and it's just one more thing when you're in a presumably stressful situation.

OK, I'm exaggerating a little about hunting through folders. Still, I've got a lot of crap on my phone. I'd bet it takes an extra second to find the right icon under duress, even if it's right on the home screen.

I've seen plenty of apps that don't seem like they'd need to load much content take an awful long time to load up. This would depend a lot on how the app is written, but there's plenty of inefficient code out there. Also, you're unlikely to have run the app recently, so it's almost certainly launching from scratch, rather than resuming from a suspended state.

So... absurd reply, maybe, but not completely absurd.
