It is technically impossible to run 15KHz arcade games at their native resolution on a regular LCD monitor.
When switchres=1 is set, MAME changes resolution to the best possible match available from a list of resolutions provided by your video card drivers. In most cases 640x480 will be the lowest res available. If you want MAME to go lower than that you'll have to make these resolutions available yourself. How to do this depends on which video card you have. The nvidia control panel has a section that lets you add new modes but your options will be limited when using an LCD display, they only really have a single resolution, any other resolution scales to full screen resulting in artifacts, or at 1:1 becomes a tiny image surrounded by black nothing. You really need a multisync CRT monitor for this.
Changing your "desktop resolution" will not accomplish anything because if switchres is set to 0 that means everything will now run at that resolution. If switchres is set to 1, it doesn't matter what your "desktop resolution" is, MAME will change resolution anyway.
