i think what you are wanting is for mame to not use the built in screen flipping code found in many cocktail games at all, but instead, if you are playing a game on eg. a real cocktail table, you start a normal 2 player game, player 1 goes first.. then when he dies, mame uses the Rotate option and simply rotates the screen 180 degrees so player 2 can now take his turn.. and then when he dies, the screen rotates back.. and so on

so basically each game would need code hacking so that the screen is rotated 180 degrees at the right point (eg. when the player dies). this is not something which will ever happen in regular mame of course, as mame is all about emulating games accurately, not adding shortcuts/hacks all over the place

so a more practical, easier alternative would be a screen flip (or rotate 180 degrees) feature which could be assigned to a keyboard/joystick button. so you play your regular 2 player game, and then when u die, you flip the screen manually yourself via a button press (which you could mount somewhere on your cocktail table). unfortunately this feature isnt in mame, but i wouldnt have thought it would be too hard to add to one of the unofficial builds
