You haven't said what your hardware is, what front end your running, what kind of joystick, what kind of interface... again, the more details you provide, the better answer you can get. Right now the best I can say is you need to read the mame faq to see if that helps.
One thought I have is mame32 is obsolete... replaced with mameUI. Also, you mention using a front end... so why are you using mame32 instead of just straight mame? I don't use mame32 or mameui, but it's possible there is a conflict between mame32 and your front end, since they're both "front ends", and they may both be trying to remap the inputs for mame. You may want to get the newest version mameui and drop the front end, or switch to straight mame for use with your front end, whatever it is. To simplify troubleshooting, try launching command-line mame until you can get it to behave as you like, then add the front end and tweek from there.
To many unknown variables about your system to really give you a good answer.
