As well as running a fair bit of security apps (Sophos, Commodo, Sypbot, Peer Guardian, Windows Defender, Firefox with max protection + Adblock, etc), nothing gets near my PC's without being first scanned by Virus Total.
VT is a web service that scans what you upload through about 42 virus and malware scanners. It's a great tool. Ofcourse, even a clean report still isn't a 100% guarantee of being safe.
If I'm not convinced and just have to run something, I run it sandboxed. (VirtualBox, Sandboxie, etc).
