is there a way to send the sound to a specific sound output, similar to the way game video can be sent to specific monitor ?
my PC pushes media center to the TV (output #3), plays MAME game on the arcade cabinet monitor (output #2), and everything else goes to the third (output #1, default) monitor.
problem is, I cannot get the MAME sound separated and sent to the arcade cabinet speakers only.

You mean MAME command-line, not MAME DOS. The "official" mame binary release is for Win32 command prompt, not DOS. In other words, it requires that you be in Windows.
MAME32 is different, it's basically the same as MAME (command line) but with a familiar Windows-style user interface.

I applied the patch to mame0111 source, and compiled successfully. The two command lines work beautifully, adding multi sound card support functionality to the Windows command line version of MAME 0.111.
To list the available sound devices: mame.exe -listsounddevices (or -lsd)
To select one of the available sound cards, eg. #2: mame.exe -sounddevice 2
... ... ... This now fully compliments the select display option, allowing anyone to route both MAME video and sound to an arcade cabinet, while sending any other "media center" contents elsewhere (living room, etc.)
Hope to see these command line options propagate onto the official binary, and further on to MAME32 Sound Tab under General Game Options.
If there is anything I can do to help make this happen, please let me know (documentation, testing, other).
