The paths may well be defined in mame.ini, but it's probably looking for mame.ini in the path where the poster is running it from and not finding it. That would be why your suggestion worked for him.

No it isn't a bug. MAME looks for mame.ini in the working directory, not the executable's directory. Windows sets the working directory to the executable's directory when you double-click it, so it finds mame.ini located alongside it. But if you run it from a different directory, it will look for mame.ini in that directory.
