Old Optipac

I have what I think is one of the original Optipacs. The reason I think it is one of the first is that it is on an I-Pac board and I was sent a paper overlay to label the connections. Does anyone know if the LED should be on when it is working properly? Mine only comes on for a second when it is first plugged in. I have been having a problem with it randomly disconnecting and reconnecting. (I get the windows sound of a USB device being unplugged and plugged back in.) When it does that it also changes the ID of the devices plugged in to it forcing me to go back and change the mouse inputs. I probably need a new one, but would like to be sure first.

That IS old. It helps to post what OS and hardware you're using - though if it's XP, the opti is probably bad.
Then again, I think the chip is removable. Rub the whole thing down with alcohol. Get a chip remover and pull it off. Inspect the pins, make sure they're all straight and everything. Clean them, clean the sockets (don't use a cotton swab for the latter), then re-seat the chip.
