Only 1 button working on i-pac 4 with win-ipac

Hi, have been toying with a new ipac 4, trying to get it set up for the first time. I have made a control panel for 1 player and have switches connected up correctly.
Loading the ipac4.ipa program, as well as creating one from scratch, only gives results for button 1 on player one. Just can't for the life of me get any other buttons/joystick to respond. Connecting up all buttons/joystick directions on my control panel to 'player 1 switch 1' on the i-pac motherboard gives a result.
I-pac connected to PC with PS/2-USB connection, does not detect when connecting via PS/2 only. Additional keyboard connected via USB for configuration.
All button configurations found in the ipac4.ipa profile match those found in MAME.
I'm just totally lost on what to do next, my last port of call is to try with a different PS/2 cable from the one supplied before I'm out of ideas.

What result does it give ?
If a "5" then the Shift function is activated. By default the other P1 buttons dont have a shift code assigned to them.
Possible causes P1 Start button shorted P1 Start button incorrectly connected Shift Lock activated on secondary keyboard ( ?????)
