Streaking in the movie Joysticks, which not only appears in the movie but actually figures into the plot in a minor way.

Starfighter.... in the movie The Last Star Fighter, although i guess since it wasnt a real game at the time i guess it doesnt count....

Bouncer in "Ninja III - The Domination"!

There's pretty much no beating that for rarity. Hell, I watched the movie when I found out Bouncer was in a scene and also figured into the plot in a minor way. 
(The movie itself is pretty terrible.)
