It actually looks like it has at least eight fingers on there. Or am I miscounting the knuckles?

The other fingers are from his (real) left hand. They're holding onto the right wrist to drive the cleaver harder into the enemy.
eta: but even that's a bit weird. :-/
eta2: At any rate... Since it is the right hand - the thumb should be on top.

He should be holding both hands together. Depending on how hard that hit is he'll easily sprain his wrist if he's not using a proper double-grip. The force of the hit has to go somewhere and even his own wrist isn't beefy enough to withstand that.
Either that or maybe he's going for the gay look afterwards, provided he gets another hit in and uses his other hand for the swinging. ;-)

EDIT: For the receiver of that strike, a good counterstrike is to go for that exposed wrist! Gimp 'im right there with the chainsaw THEN our poor fella will have a very good reason to be gripping his wrist.
