I've been doing some research on public fountains as a study on the limits for cultural acceptance in statuary themes. So I'm not looking at "avant guard" modern art crap tucked away in obscure galleries... but statues on major public fountains accepted as significant or historical and thus "legitimate" subject matter for inspiration.
What I'm gathering is... there is no difference... anything "avant guard" art has thought of to shock the viewer has already been done in the middle of town square.
Urinating naked child: check twice: Brusseles, "Manneken Pis" AND "Jeanneke Pis"
Vomiting: check: London "Vomiting Fountain"
Lactating woman spraying breast milk: check, check, check and check: Bologna, "Neptune" AND "Lady of Perpetual Lactation" Guatemala's Central Park AND Italy, Artemis' fountain (commissioned by a Catholic Cardinal) AND Tuscany, "Fountain of Women"
Masterbating and ejaculating naked man: check: Prague "Nation for itself forever"
After that last one, there's still some room to cross the pubic public line... but not as much as you would have thought.

You have to keep in mind though that in the first one, the small child has a long black hose pumping liquid forcefully into his asshole. That's the part that makes it acceptable. Take away the long black hose from his asshole and people would freak out.

Can you imagine the uproar here in the states from someone even suggesting to construct something like these?
#1 - I would think a child with a large black hose stuck up his bum would be even worse in public.
#2 - Nothing wrong with naked breasts anywhere.
Overheard in #3-
Statue # 1 - I'd say the left 1 is about that much bigger than the right. That's why it hangs lower.
#2 - We are not impressed.

I've actually been to Prague and Brussels and have seen those fountains in person. What may be a bit disturbing to some, but was apparently normal for the locals, is that on Sundays in Brussels the Manneken Pis has the water replaced with beer and you can put a glass up to it and grab a free beer. As a foreigner in the country I was shocked to see people putting a glass up to a golden liquid flowing out of a child's penis, but when it was explained to me that it was free beer, I got a glass and took my place in line. I felt kind of like Gary Glitter for a moment.
