i literally think about this 3-4 times a week. i'm not super set on the exact layout, but there needs to be 6 buttons instead of 4, and quite possibly an additional set of shoulder triggers/paddle buttons.

I also think, C and Z Buttons should be mapped like L3 and R3 buttons

As much as I would like to have an emulator-compatible controller that conforms to every legacy console controller, there are complexities that I don't want to have to deal with nowadays simply because each console was trying to differentiate. One example is the stupid design of the N64 controller. Yes, the 6-button configuration will be easier to map, but don't forget that the original controller also had buttons for capture, sync, and home which can't be easily mapped without a keyboard.

Totally 100% agree, the best controller ever released for the PC.
There were two versions though and you want the second iteration.
The first one was SV-262, that was the original 3dfx branded version. That is the one in the picture you posted. It is a translucent smoked color with a gameport connector and came with a gameport to USB adapter, it also had a battery compartment for 2 AA batteries to power the two force feedback motors.
The second one was SV-262F, it is a translucent blue color and is native USB so does not have the battery compartment. It is recognized in every version of Windows I've tried it with. They pop up on ebay occasionally usually around $50. Do yourself a favor grab one.
One other difference is the 3dfx one had a raised horizontal and vertical + design on the D pad where as the later one opted for a smoother flat contoured design. They both have right and left shoulder and trigger buttons.
