It means that game uses some kind of anti bootlegging/anti piracy feature on the original PCB that isn't understood/emulated in MAME, so certain things may be wrong. Bad graphics, won't boot, sometimes subtle things like the game will reset in later levels, or enemy AI is wrong.

In many respects, it is much like GAME_NOT_WORKING because there isn't enough confidence or there is uncertainly that you can reasonably play the game without experiencing some difficulty.
