Where do the color palettes come from?

Where does MAME actually get the correct colors for the games from? For example in older versions of MAME the games "Pac-Man" and "Mario Bros." were shown in different colors than today. But games like "Street Fighter II" have exactly the same colors, from old versions like MAME 0.104 up until today's most current version. So, how are those colors determined? And why was the palette changed for some games?

Does nobody have an idea? I know that for the NES, you have a certain number of color entries, but there are no definite absolute color values for them. If you write an emulator, you have to define yourself which entry in the palette shall have which RGB value. That's why every emulator has another palette: All of them are just defined by manually trying to simulate the colors of a TV. But an NES has a color palette of 64 colors, with the last few not even used. So, it's not such a huge work to define all colors by hand. But how is this done with games like "Street Fighter II" which can have thousands of colors? I doubt that the MAME programmers mapped each palette entry manually by observing the arcade screen. So, how are the color values read?
