The only problem with custom controllers for iPhone/iPad is support. On a computer, every program has to accept keyboard input at the very least, so a device can always emulate that. On iPads and iPhones, though, you're stuck waiting for developer support. I was looking at getting a gamepad controller (ThinkGeek is blowing one out kind of cheap), but it's the same problem. They've got support for the Atari, Activision, Midway, and Namco apps, which is cool, but other than that there's not much, and it doesn't seem like too many more developers are jumping on board.
